Puppy/Dog Questionnaire

We at EverAfter Siberians strive to ensure that all of our Siberians and their new families are truly happy together. Therefore, we require all prospective buyers to fill out this questionnaire. Please understand that each individual puppy/dog placement is determined according to that specific animal's personality and by the role that he/she will play within his/her new family. We are very thorough in screening potential homes for our Siberians so that we are confident that each placement is as perfect a match as possible. We want our Siberians to have only the best homes possible and to live happy and healthy lives. We trust that, as a dog lover, you too, can appreciate this. Our ultimate goal is that our puppy/dog becomes a permanent member of his/her new family and that he/she has the best chance of living...

Our Requirements
 

All of our Siberians are sold by contract only. We do this to protect our dogs and to assure our buyers that we stand behind all EverAfter Siberians.

 
Would you agree to signing our contract?
If no, please explain why:
 

We require that all buyers provide us with a check for $15 made out to us for the AKC registration of the puppy/dog at the time of purchase, in addition to the purchase price of the puppy/dog. We complete all puppy/dog AKC registration applications and/or transfer paperwork and have the new registration certificates mailed directly to our buyers. For our record keeping purposes, we require that all buyers provide us with a copy of the new registration certificate upon receipt from the AKC.

 
Would you agree to this registration portion of our contract?
If no, please explain why:
 

We require all of our buyers to have OFA (hip x-rays/exam and thyroid test) and CERF (eye exam) certifications done on their EverAfter Siberian and to provide us with copies of all necessary paperwork for our records. This is our way of monitoring three very important health related aspects of our breeding program.

We strongly recommend that all companion puppies attend puppy obedience class (oftentimes referred to as "puppy kindergarten"). This helps with socialization and learning manners in a structured setting. It is also a very positive way for a puppy and his/her new owner to bond and offers the opportunity for the puppy to have a scheduled weekly outing.

 
Would you be willing to fulfill this kindergarten recommendation?
If no, please explain why:
 

All dogs sold as companion animals are required to be spayed/neutered. All adults are spayed/neutered prior to going to their new homes. All puppies are required to be spayed/ neutered at their new owner's expense prior to turning six months old. All AKC registration papers will have the non-breeding (limited registration) boxes checked prior to submission to the AKC.

 
Would you agree to our spay/neuter requirement?

We require our buyers to provide regular Veterinary care for our dogs, including but not limited to annual exams, monthly heartworm preventative (we recommend Interceptor over HeartGuard), monthly flea and tick preventative, boosters of the core vaccines (NO Lepto, Lymes, Corona or Bordatella) and rabies every three years (or as required by their own state and local regulations), and any other Veterinary care to be administered promptly as needed.

 
Would you agree to our Veterinary requirement?

Our dogs are fed exclusively RedPaw, which is a fish-based Premium dog food, ideal for Northern breeds. Since RedPaw can be purchased through Authorized Dealers only, we realize that it is not readily available in all areas of the country, and therefore, understand that realistically our buyers will not be able to feed it. We do require, though, that our buyers choose another high-quality premium food. We do not allow our dogs to be fed cheap, poor-quality "grocery store brands" or those containing "meat byproducts".

All of our puppies/dogs are implanted with AVID microchips. We require that all microchips be registered in AVID's registry with our name listed as Primary Contact and the Buyer's name(s) listed as Secondary Contact. We do this to ensure that we remain "in the loop" when it comes to a potential "escape artist" and/or are notified in the case of an EverAfter Siberian being lost, stolen or running at large for any reason. We require that all buyers provide us with a check for $40 made out to us for the microchip service and registration at the time of purchase, in addition to the purchase price of the puppy/dog.

We strive to produce Siberians that are physically and mentally sound. However, by no means are our dogs perfect. If a serious issue does arise, we require that we be contacted immediately with full details, whether the problem is genetic or otherwise, including death due to any cause. Once again, this helps us to keep accurate records and to be aware of the status of the dogs we have produced.

Periodically we request updates from our Siberian buyers. We expect to hear back from them at these times, as this communication allows us to keep in touch with our dogs and helps to ensure us of their wellbeing. Again, this also assists us with our record keeping.

Note: We encourage our buyers to send updates to us at anytime throughout their Siberian's life and, of course, we love pictures! :)

 

We require immediate notification if our buyers can no longer keep their EverAfter Siberian. These dogs must NOT ever be sold, given away, surrendered to a shelter or rescue or rehomed in any manner and MUST return to EverAfter Siberians if their owners can no longer keep them.
